Labour died hit by dumper truck !!!!???


Description:
During backfilling activity at storm water 2000mm diameter RC pipeline and in absence of both flagman and supervision, a 32 years old laborer, was assigned to track number of trips and assist to ensure soil materials are dumped on the desired locations. At around11:11 AM, the laborer was hit by a reversing dumper truck when he, for unknow reason, was crossing behind the dumper truck who was on his way to dump the soil materials on location. Police & Ambulance were called as part of emergency response and unfortunately, the laborer was announced dead on spot.

Contributing Factor/Causes:
• Unsafe driving behavior (failure to make safe/check reversing camera) prior to reverse
• Absence of competent Flagmen to control the movement of machineries
• Hazard Identification / TBT for task was not completed
• Absence of direct supervisor
• Ineffective HSE Monitoring
• Sandy / Dusty Weather Condition

Root Cause:
• Lack of Resources for the task
• Planning for the work is less than adequate
• Supervision assigned for the work is less than adequate
• Approved Method Statement and Risk Assessment were not implemented
• HSE Training for the work team is less than adequate
• Lack of Medical Emergency Rescue arrangements
• Procedure for Monitoring, Supervision and Working beyond approved working hours and on
Non-Working Days doesn’t exist
• Official Approvals on Works commencing beyond approved working hours and on Non-Working
Days were not secured from Engineer/Employer

Lessons Learned:
• Resources of competent flagmen to be reviewed and match with activities involving Plants,
Machineries & Equipment’s
• Project Personnel to be re-trained on Hazard Identification Process
• Risk Assessment for work to be strictly followed and implemented
• HSE Training Program on Project to be revised/developed and closely monitored by the Engineer
• Only Competent HSE Officers to be assigned on the job
• Medical Emergency Rescue arrangements for the project to be reviewed to ensure preparedness
• Effective of HSE Inspections Program to be regularly reviewed/monitored
• Supervision Consultant to regularly conduct Thorough HSE Audit to ensure Contractor
compliance to legal and contractual requirements
• Works on overtime hours, Fridays and in public holidays shall be carried out with proper
assessment of Safety Arrangements, adequate supervision and necessary employer approvals
• “Dealing with Adverse Weather Conditions”
